Withdrawal

I/we are notifying Oakland Christian School that the student(s) listed above will not be returning for the 2027-2028 school year.

I/we understand that by submitting this form:

1. Our student's continuous enrollment at Oakland Christian School for the 2027-2028 school year will be canceled.
2. Our student's seat for the 2027-2028 school year will be released.
3. If I/we later decide that we would like our student to return to OCS, enrollment will be subject to available space and the applicable admissions or re-entry process.
4. If I/we submit this opt-out form after the February 22, 2027 Annual Withdrawal Deadline and before making our first tuition payment in May, a $750 late-withdrawal fee per student will be assessed.
5. If our first May tuition payment has already been made, the $750 late-withdrawal fee will not apply. Instead, the school's regular tuition withdrawal and refund policy will apply. I/we will be responsible for tuition paid or due through the month in which our student withdraws, as outlined in the OCS Financial Agreement.

Parent/Guardian Acknowledgment

By signing below, I/we confirm that our student will not be returning to Oakland Christian School for the 2027-2028 school year and authorize OCS to release our student's seat.

I/we understand the applicable withdrawal terms described above, including any financial obligation that may result based on the date this form is submitted.
